Medication-associated risk factors
- diuretics – dehydration may increase uric acid level and may trigger gout-attack.
- Low-dose aspirin – lowers uric acid excretion in the kidney (Buttaro et al., 2021; Li et al., 2021)
Diagnostics
Gold standard – joint fluid needle aspiration
実際に行われるか否かは別にして（痛すぎ）、試験回答としては「aspiration」。
Acute Phase Treatment
- Corticosteroid – 糖尿病の人にはダメ
- NSAID – 腎臓疾患の人にはダメ
- Colchicine – Gout attackから12-24時間以内に。腎臓疾患 (GFR<10 ml/min)の人にはダメ。GFR<50 ml/minの人にはlow doseで。
Urate-lowering therapy (ULT)を既に受けている人は、gout attackがあっても、中断したり服用量を変えたりしない。また、acute phaseにULTを始めたりしない。
Chronic Phase Treatment

Urate-lowering therapy (ULT) – 年に２回以上attackがある人はULTを開始。
- allopurinol – Uric acid overproductionの人、腎臓疾患がある人、利尿薬使用の人に。
- febuxostat – allopurinolにアレルギーがある人に。Drug interaction – toxicity may occur with azathioprine (RA med) or mercaptopurine. (chemo drug) – both immunosuppressant drugs. 覚え方としては黄色部分がどちらもプリン – People with gout needs to have a low-purine diet.
ULTと一緒にColchicineを予防的に8週間＋
