猫に噛まれた時

知り合いが指を親戚の猫に噛まれた時、早々に傷が閉じてしまって、その後、完治まで数か月もかかりました。小さな傷だったようですが、結局骨にまで行ってしまいました。

  • Cat bite – infection rate = 80-85%
  • Dog bite – infection rate = 5% (確率が低くても部位や傷の深さによってリスクはいろいろ）

Face – 顔を噛まれた場合 – extensive irrigation + ABT – primary closure -> Referral to plastic surgeon

Cat bites, human bites, deep puncuture wounds, infected wounds, 6-12時間以上が経過したwounds, and wounds to the hand ===> should be left open (due to high riks of infection) ===> deleyed primary closure or secondary intension.

Hand/Foot ===> immobilized & elevated for 1-3 days

Cat bites/Human bites ===> ABT prophylaxis for 5-7 days (例外: within 72 hours, and no/low risk of infection)

Any animal/human bite on face or hand  => physician consultation

Antibiotics

For fresh bite ==> Augmentin 500mg/125mg Q8H x5-7 days

Penicillin allergy ==> Clindamycin + doxycyclin

or Clindamycin + Bactrim (clindamycinだけだとpasteurellaに対して弱い）

他のオプション ==> levofloxacin, ciprofloxacin

Penicillin allergyでかつPregnancy ==> Macrolides
