Combined Oral Contraceptives (COCs)
Mechanism of Action
Estrogen – inhibits follicular development.
Progesterone – reduces secretion of FSH (follicle-stimulating hormone) and LH (luteinizing hormone) by negative feedback to the hypothalamus. (Cooper et al., 2022)
Components
Estrogen is pretty much Estradiol or Estetrol. Estrogenはほぼ固定。量は<50 mcg。
Progestinは種類や量が色々。
1st generation – Norethindrone acetate, Norethynodres, Ethynodiol diacetate, Lynestenol
2nd generation – Levonorgestrel, dl-Norgestrel
3rd generation – Norgestimate, Gestodene, Desogestrel
Unclassified – Drospirenone, Cyproterone acetate
A/Eが出た場合は、Estrogenは量を減らすか、protestinなら別の種類に変更可能。
Benefit of Cancer Risk Reduction
- Endometrial cancer のリスク50%減
- Ovarian cancer のリスク27%減
- Colon cancer のリスク18%減
Longer use is better (more benefit)

Adverse Effects
多くの場合マイルド。初めて使用する場合、A/Eは時間と共に解決しますが（約2~3か月）、必要なら違うものに変更すると良い。
Most common – breakthrough bleeding
Increased risk of blood clots and embolic events. というわけで、Do Not Smoke.
nausea (寝る前に飲むと良い）
Headaches
Cramping
Breast tenderness
Spotting between menses
Hypertension (HTN) (4-5% for women with no previous HTN).
Worsening HTN (9-16% for women already had HTN)
Contraindications
Smokers
Smokers ✙ 35歳以上
MIやCVAのリスクがある人
DVTやPEのリスクがある人
HTNがある人
とにかくcoagulation系が駄目。Thrombophlebitisとか。
Drug Interaction
Anticonvulsants/antiepileptics (ex. carbamazepine) と併用すると、両方の薬のメタボリズムに影響する。
Non-contraceptive reasons (避妊以外の使用目的)
COCの使用目的は、本来、避妊ですが、下記のようなコンディションにも使用されます。
- Menstrual-related disorders (pain, irregular period, fibroids, migraines)
- Acne – requires both estrogen & progesterone to suppress androgen. Progestinだけだとacneが悪化。Oral antibioticsを併用すると効果的。(ex. Tetracyclines. S/Eが少ないのはazithromycin。
- Hirsutism
Hormone Therapy for Acne Treatment:ニキビ治療用に承認されているものは下記の３つ。どれにも同じestrogenが入っていますが、Preogesteroneは全部違う種類。estrogen+progesteroneの両方が無いと駄目。Progesterone-onlyではニキビが悪化する。
Ortho Tri-Cyclen = ethinyl estradol + norgestimate (3rd generation)
Estrostep = ethynyl estradol + norethindrone (1st generation)
YAZ = ethynyl estradol + drospirenone (generation, non-classified)
Acne treatmentには、上に挙げたHormone Therapyの他に、Topical therapy (tretinoin, adapalene, tazarotene, benzoyl peroxide, salicylic acid)、Topical antibiotics (erythromycin, clindamycin, metronidazole, sulfonamide, etc.)、Oral antibiotics for inflammatory states (erythromycin, tetracycline, doxycycline, etc. )、そしてRetinoid Therapy (Oral Accutane, etc. )などがあります。
その他
Medroxyprogesterone acetate (Depot-Provera)を飲んでる場合、ovulation は2~3か月来ない。Progestin-only contraception. Injection (IM or SQ routes). IM – med can reach the blood stream quicker. SQ – benefits include the better contraceptive accessibility, reduce disparity, and increase autonomy.
